Penelitian ini bertujuan untuk mengembangkan dan menguji kualitas Instrumen penilaian berpikir kreatif (kevalidan, kepraktisan dan keefektifan) yang dilaksanakan dengan berbantuan Google Form. Penelitian ini mengadaptasi model penelitian dan pengembangan (R&D) Thiagarajan yang dikenal dengan 4D (define, design, develop, & disseminate) yang diujicobakan di kelas VI SDN Ketintang I/409 Surabaya tahun 2019/2020 dengan One-Group Pretest-Posttest Design. Teknik analisis data yang digunakan menggunakan teknik analisis deskriptif kualitatif dan kuantitatif.
Hasil penelitian menunjukkan instrumen penilaian berpikir kreatif berbantuan google form yang dibuat valid, praktis dan efektif untuk mengukur kemampuan berpikir kreatif siswa kelas VI SD. Valid dilihat dari penilaian kedua validator yang menyatakan bahwa instrumen penilaian valid dan dapat digunakan dengan sedikit revisi. Praktis dilihat dari penilaian yang dilakukan oleh guru dan respon siswa yang hasilnya dikategorikan sangat baik (tinggi). Efektif dapat dilihat dari kemampuan instrumen penilaian yang dilakukan dalam mengukur dan membedakan kemampuan berpikir kreatif siswa. Skor N-Gain menunjukkan kemampuan instrumen yang dibuat untuk mengukur perkembangan kemampuan berpikir kreatif siswa saat pretes dan posttes.
Berdasarkan hasil analisis data, dapat disimpulkan bahwa pengembangan instrumen penilaian berpikir kreatif matematika berbantuan google form yang dibuat mempunyai kualitas yang baik (valid, praktis, dan efektif) untuk mengukur kemampuan berpikir kreatif siswa SD kelas VI materi bilangan bulat, sehingga instrumen penilaian berpikir kreatif berbantuan google form disarankan untuk diterapkan dalam penilaian kemampuan berpikir kreatif siswa pembelajaran matematika di kelas VI sekolah dasar yang disesuaikan dengan kondisi dan situasi di sekolah.

This study aims to develop and test the quality of creative thinking assessment instruments (validity, practicality and effectiveness) which are carried out with the assistance of Google Form. This study adapted Thiagarajan's research and development (R&D) model known as 4D (define, design, develop, & disseminate) which was tested in class VI of SDN Ketintang I / 409 Surabaya in 2019/2020 with One-Group Pretest-Posttest Design. The data analysis technique used is descriptive qualitative and quantitative analysis techniques.
The results of the research show that the instrument for assessing creative thinking aided by google forms is made valid, practical and effective for measuring the creative thinking skills of grade VI students. Valid seen from the second assessment of the validator which states that the assessment instrument is valid and can be used with a little revision. Practically seen from the assessments conducted by the teacher and student responses the results are categorized as very good (high). Effective can be seen from the ability of the assessment instruments carried out in measuring and differentiating students' creative thinking abilities. The N-Gain score shows the ability of the instrument that was created to measure the development of students' creative thinking abilities during pretest and post-test.
Based on the results of data analysis, it can be concluded that the development of a mathematical creative thinking assessment instrument assisted by Google forms is made to have good quality (valid, practical, and effective) to measure the creative thinking ability of elementary school students in grade VI of integer material, so that the creative thinking assessment instrument is assisted Google Form is suggested to be applied in the assessment of students' creative thinking abilities in mathematics learning in grade VI of primary schools which are adjusted to the conditions and situations at school.
