Preparation of Contract Change Order Documents in the Menur Mental Hospital Integrated Building Project

Penelitian ini bertujuan untuk memberikan informasi mengenai tata cara pelaksanaan pengajuan dokumen contract change order maupun addendum pada proyek pembangunan Gedung Terintegritas Rumah Sakit Jiwa Menur Surabaya. Penelitian ini juga membahas tentang dampak dan pengaruh yang menyebabkan perubahan biaya konstruksi. Selain itu penelitian ini juga membahas tentang penyusunan standart operating procedur mengenai pelaksanaan pengajuan dokumen contract change order dan addendum. Hasil penelitian ini menjelaskan tentang alur pelaksanaan pembuatan dokumen dimulai dari peninjauan lapangan hingga penawaran harga satuan. Penyebab terjadinyan contract change order dan addendum pada pelaksanaan tersebut dikarenakan ketidak sesuaian antara gambar rencana dengan kondisi lapangan sesungguhnya. Sedangkan untuk aspek yang paling berpengaruh yang menyebabkan penambahan biaya konstruksi adalah pada penambahan biaya lembur pekerja dengan besarnya dampak terhadap penambahaan biaya yaitu sebesar 65%. Sedangkan untuk penyusunan SOP mengenai pelaksanaan pengajuan CCO dan addendum dimulai dari lembar pengesahan hingga dokumen terkait yang dibutuhkan pada pelaksanaan CCO maupun addendum.

This study aims to provide information regarding the procedures for submitting change order and addendum contract documents to the construction project of the Menur Mental Hospital Surabaya Integrity Building. This study also discusses the impacts and influences that cause changes in construction costs. In addition, this study also discusses the preparation of standard operating procedures regarding the implementation of document submission change order contracts and addendums. The results of this study explain the flow of implementation of document creation starting from field observations to unit price bidding. The reason for the occurrence of a change order contract and an addendum in the implementation was due to a discrepancy between the plan drawings and the actual field conditions. Meanwhile, the most influential aspect that caused the addition of construction costs was the addition of overtime costs for workers with a large impact on additional costs, namely 65%. Meanwhile, for the preparation of SOPs regarding the implementation of CCO and addendum submissions, it starts from the approval sheet to the relevant documents required for the implementation of CCO and addendums.
