EFEK ANTIHIPERGLIKEMIK EKSTRAK DAUN CEREMAI (Phyllanthus acidus) PADA MENCIT (Mus musculus) DIABETES MELLITUS TIPE II

ANTIHYPERGLYCEMIC EFFECT OF CEREMAI LEAF EXTRACT (Phyllanthus acidus) IN TYPE II DIABETES MELLITUS MICE (Mus musculus)

    Pemberian High Fat Diet dan induksi aloksan menyebabkan peningkatan asam lemak bebas yang memicu terbentuknya reactive oxygen species (ROS) dan resistensi insulin sehingga metabolisme kadar gula darah terganggu. Kandungan antioksidan ekstrak daun Phyllanthus acidus berpotensi dalam stabilisasi insulin. Penelitian ini bertujuan untuk mengetahui pengaruh ekstrak daun P.acidus terhadap penurunan gula darah puasa (GDP), histopatologi pankreas, serta perubahan kadar keton mencit diabetes mellitus (DM) tipe II. Penelitian eksperimental menggunakan 24 ekor mencit jantan strain Deutsch Denken Yoken (DDY) dengan metode Rancangan Acak Lengkap yang terbagi menjadi enam kelompok dengan empat pengulangan terdiri dari kelompok normal, kontrol negatif (HFD dan Aloksan), kontrol positif (Metformin), kelompok dosis I (215 mg/kg BB), dosis II (230 mg/kg BB), dan dosis III (245 mg/kg BB). Induksi HFD dilakukan selama 8 minggu dikombinasikan dengan dosis tunggal aloksan 100 mg/kg BB intraperitoneal serta 21 hari perlakuan ekstrak. Data gula darah dan histopatologi pankreas diuji normalitas dan homogenitas menggunakan uji Kolmogorov-Smirnov dan uji Levene. Data yang normal dan homogen diuji dengan One Way ANOVA. Apabila hasilnya signifikan dilanjutkan dengan uji Duncan. Data kadar keton dianalisis secara deskriptif kualitatif menggunakan pedoman warna uji pada strip tes URS-10T®. Hasil penelitian membuktikan bahwa ekstrak daun P.acidus berpengaruh sangat signifikan terhadap penurunan GDP (p=0,004) serta tingkat kerusakan pankreas mencit (p=0,008). Namun, tidak menunjukkan adanya perubahan pada kadar keton. Sehingga dapat disimpulkan bahwa ekstrak daun P.acidus memiliki efek antihiperglikemik terhadap penurunan GDP mencit DM tipe II  dalam batas normal sekaligus dapat memicu proses regenerasi sel β pankreas dibandingkan dengan perlakuan lainnya (p<0,05) dengan dosis III (245 mg/kg BB) sebagai dosis ekstrak yang optimal.

    The induction of High Fat Diet and alloxan causes increasing free fatty acids that trigger the formation of reactive oxygen species (ROS) and insulin resistance then the metabolism of blood sugar levels is disrupted. The antioxidant of Phyllanthus acidus leaf extract has the potential to stabilize insulin. This study aims to determine the effect of P.acidus leaf extract on reducing fasting blood glucose (FBG), pancreatic histopathology, and changes in ketone levels of type II diabetes mellitus (DM) mice. The experimental study used 24 DDY strain male mice in a completely randomized design, divided into six groups with four repetitions consisting of the control group, negative control (HFD and Alloxan), positive (Metformin), dose I (215 mg/kg BW), dose II ( 230 mg/kg BW), and dose III (245 mg/kg BW). HFD induction was conducted for 8 weeks combined with a single dose of alloxan and 21 days of extract. Data were analyzed in SPSS with the Kolmogorov-Smirnov and Levene's tests. Data were normally distributed and homogenous and were analyzed with One-way ANOVA, continuing with Duncan's test. The data on ketone levels were analyzed descriptively and qualitatively using the test color guidelines on the URS-10T® test strip. The results proved that this extract significantly affected decreasing FBG (p=0.004) and level of pancreatic damage (p=0.008). However, it didn't show any change in ketone levels. It concluded that P.acidus leaf extract has an antihyperglycemic effect on reducing FBG of type II DM mice within normal and also being able to trigger pancreatic β cell regeneration compared to other treatments (p<0,05) with dose III (245 mg/kg BW) as the optimal extract dose.
